הטרנספורט היוני הלא תקין בחולי CF

שלום, אנחנו תלמידות ביולוגיה שנה ג` שעורכות הרצאה בנוגע ל - CF, ורצינו לדעת מה בדיוק לא תקין בטרנספורט היוני בתאי האפיתל. בספרות מצאנו כי גם הפרשת הכלור אל הלומן וגם ספיגת הנתרן אל התאים פגומה. לגבי בלוטות זיעה הבנו כי יש הפרשת יתר של כלור וחוסר ספיגה של נתרן, ולכן הזיעה מלוחה יותר. לעומת זאת, לגבי בלוטות ההפרשה בדרכי הנשימה הבנו כי יש ספיגה מוגברת של נתרן והיעדר הפרשה של כלור אל הלומן, כך שיש ספיגת מים מוגברת ויבוש נוזלי דרכי הנשימה. האם נכון הוא שהמצב הפוך בשני תאי האפיתל השונים. מהו תפקידו של ה- CFTR בהעברת היונים ? אנא החזירו לנו תשובה במהירות, עלינו לבצע את ההרצאה עד סוף השבוע. בתודה מראש, טל והדס.

זו תשובתה של דר` חנה בלאו, אם משהו רוצה תרגום לעברית אדאג לכך CFTR appears to be a chloride channel (CAMP controlled) but it is not entirely clear what direction the transport is. The mostaccepted view is that a mutation in CFTR means that chloride cannot leave the apical membrane of the airway epithelial cells (or possibly the cells within the secretory glands of the airway). In addition, CFTR is normally an inhibitor of sodium reabsorption from the lumen into the epithelial cell, by acting on the epithelial sodium channel eNac. So CFTR mutations cause increased sodium reabsorption. This theory of CFTR function is the basis for the nasal potential difference assay, which shows changes in voltage across the cell membranes in CF compared with healthy controls.It has long been confusing that the abnormality within the sweat duct is the opposite of this. Here it appears that chloride reabsorption from the sweat duct lumen is blocked. Just to confuse you further, there are some scientists (eg. Welsh) who believe that the defect in the airway is the same as the sweat duct, and that the concentration of chloride and sodium within the airway is high, rather than low. The problem is that it is very difficult to measure the concentration of salt within the airway lining fluid. Trying, eg. with filter paper, could easily absorb electrolytes from within the cell as well. Sofisticated recent experiments seem to support the initial hypothesis, which is the one you described.
